Northamptonshire Care Record Launches EPaCCS to Improve End of Life Care Coordination Across the County

Gentleman receiving care

The Northamptonshire Care Record is proud to announce the launch of the Electronic Palliative Care Coordination System (EPaCCS), a significant step forward in improving the coordination, quality, and person-centred delivery of end of life and palliative care services across Northamptonshire.

EPaCCS enables authorised health and care professionals involved in a person’s care to securely access and share key information about an individual’s wishes, preferences, and clinical needs. By making this information available at the right time and in the right place, EPaCCS helps ensure people receive compassionate, coordinated care that reflects what matters most to them.

Integrated within the Northamptonshire Care Record, EPaCCS supports better communication between GPs, hospitals, community teams, ambulance services, hospices, social care, and other partner organisations. This shared approach helps reduce duplication, avoid unnecessary hospital admissions, and improve experiences for patients, families, and carers.

Claire Mansfield, Macmillan Palliative and End of Life Care Transformation Lead, said:

“The launch of EPaCCS within the Northamptonshire Care Record marks an important milestone in our commitment to joined-up, personalised care. By sharing vital information securely across organisations, we can better support people approaching the end of life, ensuring their choices and preferences are known and respected.”

Matthew Hutton, Digital Lead, Northamptonshire ICB, said:

“Improving digital care planning is an important part of the NHS 10-year plan. After our success introducing ReSPECT to our Shared Care Record, EPaCCS was the next logical step in improving our digital maturity further. Being able to track and care for the final wishes of a patient at this difficult time for a family is vital part of why we are digitally enabling this service across our county.”

Key benefits of EPaCCS include:

  • Improved visibility of advance care plans and patient preferences
  • Faster access to essential information during urgent or unplanned care situations
  • Better coordination between health and social care teams
  • More person-centred care aligned to individual wishes
  • Enhanced support for families and carers

The launch reflects the ongoing commitment of Northamptonshire partners to use digital innovation to improve outcomes, strengthen collaboration, and deliver high-quality care for local communities.
