Countywide Age Well teams

Our Age Well teams are community-based support workers helping older and frail people in Northamptonshire. 

These teams are based in most of our county’s Primary Care Networks (PCNs). Each PCN will vary in how their team works to suit the needs of their area. 


Why have we introduced Age Well teams?

We want to ensure that older and frail people in Northamptonshire can choose well, stay well and live well in the community. This includes improving how and when patients can access the care that they need. Our Age Well teams will, where possible, see people in the convenience and comfort of their own homes.

The Age Well teams can work with you to:

  • agree a support plan 
  • ensure you have any mobility and monitoring equipment you need at home
  • assist you in accessing local social and exercise groups to get specialist advice and guidance
  • arrange a review with a GP, pharmacy colleague, and a nurse to discuss the issues that matter to you
  • link you with a befriender if you choose
  • review the benefits you are receiving to make sure you are getting the ones that are available to you 
  • ensure that those caring for you are supported

Who will be seeing you?

The Age Well teams are made up of health, social care, and voluntary sector colleagues from existing local organisations. All members of the team can assist you and will ensure you see the right person for specialist advice.

How do I see someone from the Age Well teams?

You may be contacted directly by your local team or you can refer yourself. Alternatively, carers can refer someone they care for. If you or your carer are referring, you will also need to get in touch with the Age Well contact at your GP surgery. 


Important: The Age Well team does not operate an urgent or on-call service. If there is a need to see someone quickly for a health issue please call your GP surgery or contact 111.
 

How to refer yourself or someone else

Please email agewell.centre@nhft.nhs.uk to get in touch with the team. You will then receive an online form that has a few more details to fill in. The team aims to respond to emails within 5 working days excluding Bank Holidays. 

 

What happens next?

A member of the Age Well team will contact you to understand more about your needs or the person’s needs that you care for. If you agree then they will come to visit you at home to talk in more detail about the most important things to help with first.

This may include a virtual or in-person call with the GP lead for the Age Well team and other specialist team members. Your Age Well team member will put this in place and will be with you when the call or appointment takes place.

The Age Well team is here to help you as you become older and to offer support when you need them. This may be quite often for a while or once or twice in a few months. Each person is an individual and the support given will reflect this.

Our Community Asset Groups

Watch a short video about one of our community asset groups which is for over 65s who are living with heart failure or have had a heart attack. This group is part of our overall Ageing Well programme to support people to live well in the community.

Please note that referral to this group is by GP or medical professional only.
